Description of Services Wellness Supports provides an array of mental health services to adults, adolescents, children, and families. These services address such issues as behavior management, depression, school-related problems, anger management, anxiety disorders, adjustment disorders, relationship issues, ADHD, stress management, grief and bereavement, and more. These services include:
Psychiatric Services
A licensed, board certified psychiatrist provides evaluation, consultation, and medication management.
Outpatient Therapy
Therapists are available to provide
comprehensive clinical assessments, individual, family, and group
therapy.
These services are offered in the office or
consumer’s home. We offer services during the day and evenings
Monday through Friday, and have limited weekend availability.
ENCORE: Elder Care Management
Our elder care management services are
designed to assist aging loved ones and their caregivers in
preserving dignity, quality, and enjoyment of life through an array
of services.
These services may include a comprehensive
assessment of environmental, cognitive,
medical, social, legal, and financial needs; referral and linkage to
appropriate and quality services; advance care planning;
consultation and
coordination with healthcare professionals;
routine monitoring/friendly visits; long distance reassurance,
updates, and consultations with family members, and more.
Intensive In-Home Therapy
Intensive In-Home Therapy is a
time-limited intensive family preservation intervention intended to
stabilize the living arrangement, promote reunification or prevent
out-of-home placement for youth through the age of twenty.
This service is delivered to children in
their family’s home with a family focus.
Community
Support Services
This service is provided to individuals
who have multiple service needs and can benefit from a
comprehensive, team approach of therapeutic care. The program is
designed to provide an array of services and supports based on the
individual goals of the adult or child consumer, to improve
relationships, develop social skills, improve problem-solving
skills, and/or increase independent living skills.
